Use this form if your skater is requesting to change their current class package to a new program. Common changes are:

  1. Skaters moving from Snowplow Sam to Basic Skills

  2. Skaters moving from Snowplow Sam or Basic Skills to Aspire Figure Skating or to Hockey Skating

  3. Skaters moving from Basic Skills to Teen Classes.

    We are happy to make this change to keep everyone having fun and learning a lot on the ice!

Use this form if you would like to skate with your child in the practice end. First go to www.learntoskateusa.com to get a membership, and then complete this form.

Use this form if you have a current subscription, and you would like to end it. We you must fill out the form a minimum of 48 hrs before the first of the month to have the subscription cancelled for the next month. (I.e. to cancel for June, we must receive your request between May 1-29).
